You might have previously known Long-Term Conditions as ‘Chronic Diseases’ – these are conditions that cannot be cured but can be managed through annual reviews. See below for any conditions that would mean you get an invite for this review:

BP and Pulse Hypertension (HTN), Arterial Fibrillation (AF)
Cardiovascular Coronary Heart Disease (CHD), Congestive Heart Failure (CHF), Peripheral Arterial Disease (PAD), Cerebrovascular Accident / Transient Ischaemic Attack (CVA / TIA)
Diabetes Type 1 (DM1), Type 2 (DM2), Pre-diabetes, Gestational Diabetes
Renal Chronic Kidney Disease 1-5 (CKD 1-5)
Mental Health Bipolar, Dementia, Learning Disability (LD), Psychosis
Respiratory Asthma (if inhaler issued in last year), Chronic Obstructive Pulmonary Disease (COPD)

Review and monitoring is offered to those patients who suffer from Asthma. The surgery supports patients in the management of their condition.


Review and monitoring is offered for those patients who suffer from Diabetes, both insulin and Non-insulin Dependant. The surgery supports patients in the management of their condition together with advice on lifestyle, diet, weight and glucose testing.


The surgery provides support to those patients who suffer with High Blood Pressure and Heart Problems. The surgery provides regular monitoring and testing, advice, diet and healthy lifestyle.
