To register with the practice contact Reception who will provide you with the necessary forms and Practice Leaflet. Please bring your medical card when registering, if you do not have your card then you can complete our Patient Registration Forms. You can also download the forms below, GMS1 and our New Patient Registration Form, once completed please bring to the surgery.
In order that we can establish your current health all newly registered patients should make a new patient appointment with the Doctor, as it normally takes a few weeks for your previous medical records to be forwarded to us. A urine bottle will be given to the patient to bring a sample when attending the health check.
You will be registered with the practice as a whole and will be assigned to the list of one of the partners. When making an appointment you may request a consultation with the Doctor of your choice.
We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.

If you wish to, you can now use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and look at your medical record online. You can also still use the telephone or call in to the surgery for any of these services as well. It’s your choice.
